It would be useful to be able to have a record of a discussion between the Chair and CEO that occurs not during a meeting but does have an outcome recorded as a decision.
Perhaps this could be found under Between Meetings -and just like a Flying Minute have a decision created with the outcome. This decision can also be entered into the minutes as required.
